CIP is seeking a candidate with a BSW/related degree to provide social services for homebound older adults in Northwest Philadelphia. The IHSPís goal is to help older adults remain safely in their own homes. The Intake Social Worker receives all incoming referral calls to the IHSP; conducts telephone interviews to determine consumerís appropriateness for the IHSP; responds appropriately to crisis/emergency situations within program parameters; provides information and referral to members of the community seeking assistance; resource development; maintains and builds on community relationships; maintain and reports on intake statistics; assigns cases to IHSP social workers; and performs outreach visits to community agencies as needed. Additionally, the Intake Social Worker maintains a caseload of IHSP clients.
Social Workers are required to conduct home visits therefore a car and insurance is necessary. Position is Full Time.

Work with Center in the Park staff to promote and deliver quality physical fitness programs to older adults (55+). Design and conduct individual and group exercise activities and orientation to various exercise equipment. Participate in Center-wide activities to promote physical fitness and healthy aging. Facilitate the development of individual fitness goals, action plans and outcome measurements. Provide information and referral for nutrition, weight management and related physical fitness activities. Promote the concept of healthy aging and physical fitness. Assist older adults in developing new skills and provide opportunities for socialization and recreation with their peers while building strength, endurance and physical flexibility. Experience career launching opportunities in the field of aging. Work with agency preceptor and faculty member to develop project for the academic year.
Education Requirements: Current undergraduate student or Bachelorís Degree recipient
